Third Annual Kelleys Island 3 On 3 Basketball Tournament Print E-mail
Benefit Basketball Tournament to be held at Village Pavilion Basketball Court on Addison St on Saturday, July 16, 2011 Deadline to Register: Friday, July 15, 2011. Cost per Team: $80 Register at the Winery, by mail, or pre-register using the form below! Be at the Pavilion by 9 a.m. EDT at the LATEST!

Cost includes T-shirt. Each team must consist of 3 or 4 players. Teams can be all single sex or co-ed. There will be three divisions this year: 18 and younger, 19 - 35 and 36 and older. The youngest age on the team will determine which division team is to play in. Games will be until a team scores 7 points, win by 1 or for 20 minutes. If there is a tie, 2 minute overtime. Additional rules will be distributed on tournament day.

I know that participating in strenuous activities such as basketball is potentially hazardous. I assume all risks associated with playing in this event, including, but not limited to falls, contact with other participants, effects of weather, and the condition of playing surface. All such facts being know and appreciated by me. Knowing these facts, and in consideration of your accepting my entry, I hereby for myself, my heirs, executors, administrators or anyone else who might bring claims on my behalf, covenant not to sue, and waive, and discharge all volunteers, and sponsors, including their agents, employees, assigns or anyone acting on their behalf, from any and all claims of liability for death, personal injury, and property damage of any kind or nature whatsoever arising out of or in course of my participation in this event. This Release and Waiver extends to all claims of every kind of nature whatsoever, foreseen, unforeseen, known or unknown. Application of minors will be accepted only with a parent or guardian’s signature and must also be signed by the minor.
